The ARTIFICE study (Outpatient Stroke Unit Care for Older Adults) is investigating whether outpatient care for patients with a mild stroke or a transient ischemic attack (TIA) can be a safe and effective alternative to inpatient care in a stroke unit.
The Heidelberg University Department of Neurology is conducting the study in collaboration with the Geriatric Center at Heidelberg University Hospital and inav – the Institute for Applied Health Services Research. The project is receiving 2.4 million euros in funding from the Innovation Committee of the Joint Federal Committee (G-BA).

Background

In Germany, inpatient care in a stroke unit is the gold standard for acute care of stroke patients — regardless of the severity of their condition. However, not all patients require several days of hospitalization. An outpatient alternative could offer benefits, particularly for older adults aged 60 and older, who are at the highest risk for stroke.

Objective of the Study

ARTIFICE aims to establish an outpatient stroke unit that provides care equivalent to that of inpatient treatment. The study examines the safety and effectiveness of outpatient treatment, patients’ quality of life and satisfaction, and the cost-effectiveness of the new care model.

Methodology

• Study Design: Randomized controlled trial with 400 participants aged 60 and older
• Target Population: Patients with a mild stroke or TIA who are medically stable
• Evaluation: Clinical endpoints (neurological deterioration, recurrent strokes), quality of life, and patient satisfaction (at 7 days, 3 months, and 12 months); health economic analysis

Potential

The ARTIFICE study has the potential to revolutionize stroke care for older adults. Establishing an outpatient stroke unit could enable needs-based, patient-centered care that improves both the quality of life for those affected and the efficiency of the healthcare system. Initial results are expected by the end of 2029, following completion of the data analysis.
